Take a Philips screwdriver and stick it in the rubber plug just far enough that you can pry sideways and get traction against the inside of the bar, but not so far that the tip would lodge against the bar. Pry, reset, repeat. It will normally start to work out within a few tries. That's the easy way and usually works in a few seconds.
If not, squirt a shot of WD-40 through the plug center up inside the bar, and go find some kind of metal hook like a doubled coat hanger, work it up through the hole, and pull. Usually works, but messy and time-consuming.
Last resort, get a big wood screw, thread it into the plug, apply vice grips, and work it out. Very time consuming and don't pull the bike over on yourself.
Don't forget to get the loose nut out when you're done, or it'll rattle around in there and drive you insane.
